Once you have read, filled-in and completed the agreement below, you must reach out to SWANN Billing Office to inform the staff of your chosen electronic payment method and account information; as needed, SWANN Administrative Staff will reach out to you to discuss the account information associated with your choice(s) for electronic payments.

Electronic agreement statements submitted without communicating your needs and choices with Agency personnel, prior to or immediately after submitting this form, may render the request disregarded.
